The effects are strongest while you are listening to the tones because your brainwaves are synchronized and tuned into the frequency range you desire at that time.  After you've stopped listening the effects can still linger for a while afterwards.  The timescale will vary from person to person and be affected by what you do after you've stopped listening.

The exact technique that will be right for you is something you have to discover for yourself though. There is no universal way to pinpoint which technique is best for you with 100% accuracy. You may need to try out several until you find the one that really resonates with you. One way is to do a bit of research on a variety of techniques. Usually you will find one or more of them seem to “draw” you to them. Pick one of those techniques as your starting point.
Binaural beats change the frequency of your brainwaves, giving you control over which category you experience at any given moment. And because you’re in the driver’s seat — and producing specific frequencies to induce a specific state of mind — you can use binaural beats to boost performance, increase focus, get better sleep… the possibilities are endless. “There’s an infinite number of variations on how you could use this kind of technology,” says Bill Harris, Director of Centerpointe Research Institute and creator of auditory brainwave training program Holosync.
Generally speaking, the brain will usually entrain to the strongest stimulus which would be isochronic tones over binaural beats. So when you see people add binaural beats at a different frequency to the isochronic tones, that would not produce additional brainwave entrainment at another frequency. If they are both at the same frequency I haven’t seen any research to indicate whether that would be beneficial or not, but my belief is that it would weaken the potential for entrainment. When you look at the waveform of an isochronic tone there is a distinct empty space between each beat, making it very pronounced and effective. When you add binaural beats at the same frequency it looks like this: The depth of the waveform is now half as deep and less effective. This is before the binaural beats are formed inside your head, where the waveform is hard to determine and measure. From listening to that type of combination the beats sound much less pronounced, which has to make them much less effective in terms of a brainwave entrainment stimulus, compared to isochronic tones on their own.
Research shows that listening to soothing music can lower blood pressure, heart rate, and anxiety. “Create a playlist of songs or nature sounds (the ocean, a bubbling brook, birds chirping), and allow your mind to focus on the different melodies, instruments, or singers in the piece,” Benninger says. You also can blow off steam by rocking out to more upbeat tunes -- or singing at the top of your lungs!
It takes slightly more time to practice guided imagery, but this is a great way to leave your stress behind for a while and relax your body. Some find it easier to practice than meditation, as it allows more engagement of the conscious mind. You can focus on imagining relaxing environments you would love to visit, or on memories of your "happy place" for when you're stressed. You can play natural sounds in the background as you practice, to promote a more immersive experience.

Insomnia and PTSD for most of my life are real bears that I wrestle with every night. A brain neural-therapist (for lack of a better description) said that this kind of sound recording may help. It is not music. When I remember to play the disc, I fall asleep within the first 5-8 minutes. One day I played the disc while cleaning and I was very surprised to hear how much of the recording I never recall hearing before. That's why I know how long it takes to fall asleep.
Please note: When you take one earphone out, move it as far away from your ear as possible. With some headphones you may still be able to hear the pulsating sound if the removed headphone is still fairly close to your ear; this is because your brain can still detect the frequency vibration coming from the headphone. Entrainment is a phenomenon seen is a wide variety of different natural circumstance. In the field of chronobiology, the study of biological rhythms in living organisms, entrainment is the synchronization of a circadian rhythm with the rhythm of an external pattern, such as the synchronizing of women’s menstrual cycles with the phases of the moon or with one another in a group.
“When you play two separate frequencies in each ear, say 140 Hz and 149 Hz, the difference in frequency is 9 Hz. Your brain compensates for this difference, and a third tone of 9 Hz is experienced,” says Niraj Naik, founder of Trpynaural, a resource for binaural tracks. “This will result in your brain being raised or lowered and tuned into the desired frequency. This is how binaural beats are produced.”
OK – so some of this is pretty common marketing hype. Consumers have become almost numb to such hyperbole. But still we see some of the common – almost ubiquitous – elements of pseudoscientific scams. The company claims that the device is “effort-free”, that the results are “rapid and long-lasting,” that it is the “most effective” method of its kind, and that it is useful for a broad range of applications (hey, why limit the market). I am used to such hype about dishwashing detergent, but find it intolerable when applied to a pseudoscientific device with medical applications.

While visual entrainment is more powerful than audio alone for inducing desired brainwave states, more caution is needed when using visual methods. Between 0.3 and 3 percent of the population is susceptible to having seizures from flickering light stimulation, and for other people, flickering visual stimuli simply makes them uncomfortable. People with epilepsy have a greater chance of having a seizure from exposure to flickering light stimulation.

Writing or talking about the things that prey on you—in a diary, with friends, in a support group or even a home computer file—helps you feel less alone and helpless. One study, published in The Journal of the American Medical Association, looked at people who had either rheumatoid arthritis or asthma— conditions known to be stress-sensitive. One group chronicled in a perfunctory manner the things they did each day. The other group was asked to write daily about what it was like, including fears and pain, to have their disease.
